Default Is PHPMagic what I need?

I'm not a complete novice at building websites and have even managed to install a vBulletin forum also.

Here's what I need to do...
  1. Create an online form with results saved to mysql data base
  2. Create at least a dozen accounts for employees (login and password for each account) this form will not be available to the general public.
  3. I need to have all accounts (up to 12) filling out these forms and saving the results simultaneously
  4. Ability to query partial or full contents of database and print these results
  5. Limit different accounts with different levels of access (read only)
  6. This isn't critical but the ability to convert the contents of the database to Microsoft Excel or Access.


I'm trying to help with the development of a system that will track persons evacuated, by our local Government, in times when a Mandatory Evacuation is issued. While most people have the recourses to fend for themselves there are many who will need our assistance in evacuating, We need to track everyone we've evacuated and where they were evacuated to.

If this isn't the software I need, any help pointing me in the right direction will be greatly appreciated
