Duplications of the form in email

Hi,

I've just bought the product and set up a form with a few fields which will be saved to my database and sent to me via email.
The database insertion is working perfect, but in the email there are a lot of duplications (up to six) of the form:
e.g
name: aa
surname: bb
street: cc
city: dd
name: aa
surname: bb
street: cc
city: dd

Any idea why?
